Register As A New Patient



Important: Notice

Please note that we only accept new patients who live in our published boundary (see map below)

Check you are within the practice's catchment area:

Each GP practice has a defined area within which they will accept new patients. 

To join the practice as a new patient your address must be inside the area marked in blue on the map (our inner boundary)

[The area marked in red represents our outer-boundary. Existing patients who move to a new address within the outer boundary are able to stay registered and enjoy continuity of care.]

 

How to request a registration form

All persons who wish to make an application to join our Practice List must do so by requesting an application form from our reception staff. Our Practice boundary lies between Carr Lane and extends to Boundary Street - you can check if you are in our catchment area with your postcode on our practice catchment area page.

Upon full completion (one form for each family member) the registration form should be returned along with the GMS1 form to reception along with proof of address. If it is deemed necessary to carry out a health care assessment then the applicant will be offered an appointment with our Practice Nurse or Health Care Assistant.  

The Partnership reserves the right to remove abusive and violent patients from its list and will do so in writing and in line with Health Authority policy.

In accordance with current legislation patients who wish to access their medical records may apply to do so by writing to the Practice Manager. 

If you are staying away from home and need medical support

Important: NHS 111

NHS 111 can direct you to the best place if you think you need medical help right now – you can call 111 or use NHS 111 online.

If you are staying temporarily away from your normal UK address or if you are a visitor from outside the UK you can register temporarily with this surgery. 

This is to help you if you need medical help with a new or existing condition that cannot wait until you return home.

You will need to be staying at an address that is inside the area marked blue on the map on this page.

This applies to people who are temporarily at an address inside our boundary for between 24 hours and 3 months.

You will need to call the surgery to register as a temporary resident.

Support from your usual GP

Remember that you can often get support over the telephone from your usual GP surgery. If you live in England and you are staying temporarily in another part of England, your usual GP can send prescriptions electronically to a pharmacist in this area.
